“If I live in Spain on a Non-Lucrative Visa, can I use the public health system?”

After a few years in Spain, many people start to hear completely different versions:

  • “You are never eligible, you haven’t paid into the system.”
  • “After 5 years’ residency you can join.”
  • “You need 10 years.”
  • “You can join now if you pay a monthly fee.”

1. NLV ≠ automatic access to public healthcare

The Non-Lucrative Visa is for residents who live in Spain with sufficient resources but do not work in Spain or pay into Social Security.
Therefore:

  • Holding an NLV does not create automatic entitlement to free public healthcare,
  • Not even after 5 or 10 years of residency.

The Convenio Especial de prestación de asistencia sanitaria allows foreign residents who have not contributed to Social Security to join the public health system by paying a monthly fee.

You must be:

  • A legal resident in Spain (with a valid TIE),
  • Registered on your local padrón,
  • Not otherwise entitled to SNS healthcare (employment, S1, etc.).

Approximate monthly fees:

  • Under 65: around 60 € / month
  • 65 and over: around 157 € / month

Once accepted, you use the public system like any other resident.
